Mounjaro (active ingredient: tirzepatide) acts on two gut hormones - GLP-1 and GIP.

Both hormones send fullness signals to the brain, reduce appetite and slow stomach emptying.

With the right dose, the result is steady, sustainable appetite reduction and gradual weight loss.

However, if your body hasn’t adapted to a particular dose or hasn’t had enough time to do so, you risk facing several side effects such as nausea, vomiting, and constipation.

Gradual dose escalation gives the gut and the body time to adjust to the medicine.

The four-week gap between dose escalations isn’t a random call.

It's the interval the SURMOUNT-1 trial used to allow side-effect symptoms to settle before introducing a stronger signal. The Mounjaro UK SmPC adopts the same interval as standard.

Going up too quickly on Mounjaro dosages typically does two things.

It intensifies the side effects harder, so you're more likely to stop treatment.

Plus, it doesn't meaningfully speed weight loss, because most of the appetite reduction occurs within the first few weeks at any given dose.

In the long run, increasing your Mounjaro dose faster than advised will only make you more likely to discontinue Mounjaro due to side effects, and that’s not our goal.

Here is the standard UK Mounjaro dosage schedule for weight loss.

Your prescriber may ask you to stay on the same dosage if you're tolerating it well and losing weight, switch to a higher dosage if the current dosage isn't delivering the intended results, or switch to a lower dosage if the current dose is too aggressive.

2.5mg

The introduction dose

The job of 2.5mg is to help your gut and your body adjust to tirzepatide and the common side effects that come with taking tirzepatide.

Most people lose little or no weight in these first four weeks, and that's expected.

If you feel almost no changes at 2.5mg, it doesn't mean the medication won't work for you.

The real, visible changes begin at the 5mg dose.

5mg

The first therapeutic dose

This is where the physiological changes begin.

At 5mg, you will start to feel that your appetite is reducing, that the initial side effects are fading, and that you are feeling full for longer after eating.

Many people lose weight steadily at 5mg and stay on it long-term.

The SURMOUNT-1 trial showed that the 5mg dose produced an average weight loss of approximately 15% at 72 weeks in adults with obesity.

That's a clinically meaningful result.

If you're at 5mg, losing weight steadily, and tolerating the dose without side effects that interfere with daily life, there's no clinical reason to increase the dose.

The lowest effective dose is the goal.

7.5mg & 10mg

The common maintenance doses

Based on the progress you are making so far, your prescriber might increase your dosage to 7.5mg or 10mg.

At this point, you will see visible changes in your body weight and health. Your clothes might feel loose because you've dropped a size or two.

You might also feel more energetic than before, able to perform mundane tasks with ease.

If you tolerate the doses well and are losing an adequate amount of weight, your prescriber might make the 7.5mg or 10mg dose your maintenance dose.

SURMOUNT-1 showed that the 10mg dose produced an average weight loss of 19.5% at 72 weeks, close to the 20.9% weight loss seen with the 15mg dose after 72 weeks.

12.5mg & 15mg

The higher doses

These doses are for two specific situations.

First, when 10mg has been well tolerated for several months, but your weight loss has plateaued, and you still have meaningful weight to lose.

Second, if you are a type 2 diabetic who needs a stronger glycaemic control alongside weight loss.

The 15mg dose produced the highest average weight loss in SURMOUNT-1, which was around 20.9%.

It also tends to be the dose where gastrointestinal side effects are most persistent.

Some people tolerate 15mg well; some find it less manageable than 10mg and revert to their last dose.

When to increase, decrease, or stay on the same dose

The prescriber considers three factors at each decision point: how the current dose has worked, how well you've tolerated it, and your goal.

Increase your dose when:

  • You've been at the current dose for at least four weeks
  • Your weight loss has slowed or plateaued, and you're not yet at your target weight
  • Your prescriber agrees that the next dose is suitable based on your medical history
  • You feel ready. Mounjaro isn't a race, and you have a say in pacing

Stay at the current dose when:

  • You're losing weight at a steady rate, and there's no clinical reason to dial up the dose
  • You've reached your target weight loss and want to maintain it
  • You're tolerating the current dose well and managing your day-to-day life comfortably, so you prefer not to make any new adjustments.

A useful benchmark: If you're losing roughly 0.5–1% of your body weight per week on average over several weeks, the dose is doing its job. There's no need to increase the dose.

Decrease your dose when:

  • Gastrointestinal side effects haven't eased after three to four weeks on the new dose
  • Nausea or vomiting is severe enough to affect eating, hydration, or work
  • You feel the dose is too aggressive for your appetite — you're under-eating to the point of fatigue
  • A previously-tolerated lower dose felt like a better balance of effect and comfort

Stepping back down is not a failure. It is absolutely normal to decrease your dose if it is not working for you.

What to do if you miss a dose

What to do when a week doesn't go to plan, per the Mounjaro UK SmPC.

If you miss a dose

  • If less than 4 days late, take the missed dose as soon as you remember and continue on your usual day next week.
  • If more than 4 days late: skip the missed dose entirely. Take your next dose on your usual day.
  • If you've missed multiple doses for more than one consecutive week, message your prescriber before restarting. You may need to step back to a lower dose to reintroduce treatment gently.

Don't double up to "catch up". Taking two doses close together significantly raises the risk of nausea and vomiting.

Changing your injection day

You can change your weekly injection day, provided there are at least 3 days (72 hours) between the old and new days.

For example, if you usually inject on a Monday and want to move to a Friday, you can do that in one move.

Moving from Monday to Wednesday means waiting an extra week before the change is safe.

A step up: Take the new dose at the next scheduled injection.

A step down: Take the lower dose at the next scheduled injection.

Most people find that the side effect intensity drops within one or two weeks at the lower dose.

A pause: If you stop Mounjaro for more than two weeks, our prescriber typically recommends restarting at a lower dose (often 2.5mg or 5mg) and re-escalating, because your body needs to readapt to the medicine after a pause.

The exact restart dose depends on how long you've been off and how you previously tolerated treatment.

Is your current dose right for you

If you have been following your Mounjaro dosage schedule perfectly, here are some signs that you are on the right dosage:

  • Sustained appetite reduction. You feel full earlier and stay full longer, but you don’t suppress your hunger so much that you forget to eat or struggle to take in enough calories to function. Food noise quiets down as well, and you notice you aren’t thinking about food as much.
  • A trajectory of roughly 0.5–1% body weight loss per week over several weeks. Faster weight loss isn't necessarily better and often comes with stronger side effects.
  • Side effects you can manage. Mild nausea on injection days, occasional reflux after large meals, and manageable constipation - these are typical at most doses, but they do not dominate your week.
  • Stable energy for daily life. Some fatigue around dose changes is normal, but you usually find yourself energised enough to go about your day.
