Short answer

Mounjaro (tirzepatide) is not licensed in the UK for prediabetes.

It is licensed for weight management and for type 2 diabetes.

However, prediabetes is listed as a medical condition that can, alongside a qualifying BMI, make a person eligible for Mounjaro.

So, if you have prediabetes and a BMI of 27 or above, a prescriber can deem you eligible for Mounjaro.

NICE has not issued a separate recommendation to use Mounjaro for prediabetes by itself.

However, clinical trials show that in people with obesity and prediabetes, tirzepatide reduced the risk of going on to develop type 2 diabetes by 94%.

If you have just been told you have prediabetes, it is normal to feel unsettled.

But let us reassure you, prediabetes is a warning sign, not a diagnosis of diabetes, and for many people, it is reversible.

What is prediabetes

Prediabetes means your blood sugar is higher than normal, but not high enough to be classified as type 2 diabetes.

Prediabetes is usually tested with an HbA1c blood test, which reflects your average blood sugar over the previous two to three months.

NICE UK uses these thresholds to measure if a person is non-diabetic, pre-diabetic, or diabetic.

Most people with prediabetes have no symptoms at all, which is why it so often turns up by surprise in a routine test.

Here's the reassuring part

A prediabetes result is not something to be scared of. The result is best read as useful early warning, not bad news.

It tells you where things are heading if nothing changes, and it gives you time to change them.

Many people in this range bring their blood sugar back to normal through weight loss and lifestyle + diet changes.

That said, prediabetes is worth taking seriously.

Without any change to weight, diet, or physical activity, a prediabetic individual may go on to develop type 2 diabetes later on.

Is Mounjaro licensed for prediabetes?

Mounjaro is not licensed for prediabetes as a condition in its own right. There is no UK licence that says "use tirzepatide to treat prediabetes."

So if prediabetes is the only thing on your health record, that alone might not help you become eligible for Mounjaro.

But prediabetes is listed as a qualifying condition, along with weight loss, for Mounjaro.

According to NICE guidelines, the medicine is licensed for weight management in adults with a BMI between 35-39.9 and at least one weight-related health condition or with a BMI between 32.5-37.4 with adults from South Asian, Chinese, other Asian, Middle Eastern, Black African, or African-Caribbean ethnic backgrounds.

According to Eli Lilly’s marketing authorisation indications, the medicine is licensed for weight management in adults with a BMI of 30 or above, or 27 or above with at least one weight-related health condition.

The licence lists examples of those conditions, and prediabetes is one of them, alongside high blood pressure, raised cholesterol, sleep apnoea, cardiovascular disease, and type 2 diabetes.

What that means in practice:

If you have prediabetes and a BMI of 27 or above, you might be eligible for Mounjaro.

A prescriber is not prescribing "for prediabetes" as such.

They are prescribing for weight management in someone whose prediabetes is part of the clinical picture.

Did you know?

Many people with prediabetes also have a higher BMI, since overweight or obesity can be a risk factor for prediabetes.

If your BMI is below 27, though, just having prediabetes does not make you eligible for Mounjaro.

Prescribing it in that situation would be off-label, and that is not how we operate at OnlineClinic.

What the NICE guidelines say

Here is what the NICE guidelines outline about Mounjaro and prediabetes.

NICE recommends tirzepatide for weight management.

NICE recommends tirzepatide for weight management for people with:

  • A BMI between 35 and 39 and above with at least one weight-related health condition, or
  • A BMI between 32.5 and 37.4 with at least one weight-related health condition for people from South Asian, Chinese, other Asian, Middle Eastern, Black African, or African-Caribbean ethnic backgrounds.

However, according to the marketing authorisation indications for Mounjaro (tirzepatide) from its manufacturer, Eli Lilly, tirzepatide is recommended for weight management for people with:

  • A BMI of 30 and above or 27 and above, along with a weight-related health condition, such as:
    • Hypertension
    • Dyslipidaemia
    • Obstructive sleep apnoea
    • Cardiovascular disease
    • Prediabetes
    • Type 2 diabetes mellitus

NICE has not issued a recommendation to use tirzepatide for prediabetes.

There is no NICE guidance that treats prediabetes as a standalone reason to prescribe Mounjaro.

NICE recommends it for weight management, and people with prediabetes, along with a qualifying BMI, can become eligible for treatment.

For prediabetes without obesity, NICE's approach is lifestyle-first.

NICE guidance on prediabetes centres on changes to diet, physical activity, and weight, together with regular HbA1c monitoring.

Medication is considered in specific circumstances rather than as the automatic first step.

The one GLP-1 medicine NICE has recommended in a prediabetes-adjacent setting is liraglutide, and only under strict criteria (a much higher BMI and additional cardiovascular risk factors).

So, the overall NICE guidance comes down to this: NICE backs tirzepatide for weight management, including people with prediabetes, but it does not tell prescribers to use it for prediabetes alone.

The decision always rests with the prescriber, who considers your medical history before taking a call.

A 3-year clinical trial on Mounjaro and prediabetes

Post the SURMOUNT-1 trial, another study analysed tirzepatide’s (the active ingredient in Mounjaro) safety and its effectiveness for weight loss, as well as for preventing progression of type 2 diabetes over 3 years (i.e., 176 weeks).

What the 3-year results show

  • Out of 1032 participants, only 1.3% participants taking tirzepatide went on to develop type 2 diabetes, as against 13.3% participants taking a placebo.
  • 17 weeks after stopping the treatment, only 2.4% participants taking tirzepatide developed type 2 diabetes, as against 13.7% participants taking a placebo.
  • Over 176 weeks, participants lost between 12% and 20% of their weight while on tirzepatide.

How does weight loss help prevent type 2 diabetes

Prediabetes is largely driven by insulin resistance, where the body stops responding to insulin as well as it should, often linked to carrying extra weight, especially around the belly.

Lose a meaningful amount of that weight, and the body's response to insulin tends to improve, which brings blood sugar back down.

Mounjaro works by mimicking two gut hormones (GLP-1 and GIP) that reduce appetite and slow digestion, so you eat less without a constant fight against hunger.

The weight loss that follows is what improves your blood sugar.

Lifestyle still comes first

Making lifestyle changes is an essential step to prevent prediabetes from progressing to type 2 diabetes.

Here are some practical, day-to-day changes you can incorporate into your daily routine to manage prediabetes before it turns into a concern:

  • Move more, in whatever way you can. Regular activity improves insulin sensitivity on its own.
  • Aim for gradual, sustainable weight loss rather than a crash.
  • Get your HbA1c rechecked on the schedule your prescriber suggests, so you can see whether things are moving in the right direction.

Note:

For overweight or obese individuals with prediabetes, Mounjaro is an additional support system that can make the lifestyle changes easier to implement.

For some people, the appetite reduction is what finally makes sustained weight loss possible after years of trying.

That is a legitimate reason to consider it, and nothing to feel awkward about.

But it is not a substitute for the necessary lifestyle changes.
