Foundayo: What to expect from Eli Lilly's new weight loss pill

First, there was the Wegovy pill by Novo Nordisk, which was approved by the MHRA in June 2026. In just two more months, we now have another GLP-1 pill by Eli Lilly, called orforglipron, or Foundayo.

On 10 August, 2026, the MHRA approved the orforglipron oral pill manufactured by Eli Lilly, naming it Foundayo, for managing obesity and type 2 diabetes. A couple of weeks later, Foundayo was available at private clinics and pharmacies, including OnlineClinic.

It only targets one hormone pathway, not two. Mounjaro works on two hormones, GLP-1 and GIP, which is why it tends to produce the highest average weight loss compared to currently available options. Foundayo, however, works on GLP-1 alone, which is why Mounjaro and Foundayo aren't expected to produce identical results.

It's genuinely new as a type of medicine. Foundayo is the first tablet of its kind, a small molecule that doesn't need to be built the same way as older GLP-1 medicines. This medicine allows you to skip the food and water restrictions that come with the Wegovy pill.

To understand how Foundayo works, you need to understand a few basics first: GLP-1 is a hormone your gut already makes naturally, after you eat, to tell your brain you've had enough and to slow down digestion a little.

GLP-1 medications are designed to mimic that signal more strongly and for longer.

Foundayo does this in the form of a pill rather than an injection, which sounds like a small technical difference, but is actually the tricky bit scientists have been trying to solve for years.

Most hormone-mimicking drugs are proteins, and proteins get broken down by stomach acid before they can do anything useful, which is exactly why Mounjaro and Wegovy have to be injected in the first place.

Foundayo gets around this by being built differently, as what's called a small molecule rather than a protein.

Practically, that means it survives your digestive system intact, gets absorbed, and does its job, all without needing an injection or the special coating that the Wegovy pill relies on.

While the Wegovy pill has to be taken first thing in the morning, on an empty stomach, with a small sip of water, waiting for another half an hour before eating, drinking anything else, or taking other medicines, Foundayo is easier to take.

• Take it once a day, any time that suits you
• Take it with or without food
• No special water rules, no fasting, no 30-minute wait
• Do not crush, split, or chew the medicine
• Do not take more than one pill a day
• If you miss taking your pill for more than 7 days, talk to your prescriber about it

Foundayo is introduced gradually, in six steps, starting from 0.8mg and going upto 17.2mg. The dose increases at least once a month as your body adjusts.

Foundayo belongs to the same family of medicines, and the side-effect pattern reflects that.

The side effects reported most often in the trial were:

• Nausea
• Constipation
• Diarrhoea
• Vomiting
• Indigestion or heartburn
• Headache

Foundayo is intended for adults who meet the standard UK eligibility criteria for weight loss medication, and who are looking for a pill rather than a weekly injection.

To be eligible for Foundayo, you'll generally need:

• A BMI of 30 or above, or
• A BMI of 27 or above, with at least one weight-related health condition, such as type 2 diabetes (or prediabetes), high blood pressure, high cholesterol, obstructive sleep apnoea, or cardiovascular disease

A prescriber will also want to understand your full medical history, your current medications, and what has led you to consider treatment now before agreeing that Foundayo is the right fit.

Beyond the formal criteria, Foundayo tends to suit people who:

• Would rather take a pill than inject themselves, or have found needle anxiety a real barrier to starting Mounjaro or Wegovy
• Have tried the existing Wegovy pill and found the fasting and water rules hard to build into daily life

Mounjaro currently has the strongest trial results of the four options.

Foundayo's strength is convenience, not a bigger number on the scale.

Foundayo is not suitable for anyone under 18.

As with all GLP-1 medications, treatment is for adults only.

Foundayo isn't suitable for everyone, and a prescriber will always check your full medical history before agreeing to it. You shouldn't take it if you:

• Are pregnant, planning to become pregnant, or breastfeeding
• Have a personal or family history of medullary thyroid carcinoma, a rare thyroid cancer
• Have Multiple Endocrine Neoplasia (MEN) syndrome type 2
• Have had a serious allergic reaction to orforglipron or any of its ingredients

You'll need a proper conversation with a prescriber if you have a history of pancreatitis or gallbladder problems, severe kidney or liver issues, an eating disorder, or if you're already taking insulin or other diabetes medication.

The National Institute for Health and Care Excellence (NICE) is separately reviewing whether Foundayo should be recommended for weight management.

Whatever that guidance says, it won't change anything overnight. It's simply the next step in a longer regulatory process.
