This article explains what retatrutide is, what the evidence shows so far, and why the products being sold online right now are a real risk to your health.
Before you begin: A little bit about OnlineClinic

Retatrutide is an experimental once-weekly weight loss injection developed by Eli Lilly, the company that makes Mounjaro.
It works on three hormone receptors at once:
That's why it's described as a "triple agonist". You may also see it called "GLP-3" online. However, Eli Lilly has said that isn't a scientific term.
The clinical trial results are promising. This is also a reason behind the increasing demand for illegally sold Retatrutide.
Eli Lilly has conducted multiple trials in multiple phases to test the efficacy of Retatrutide. Here’s a detailed breakdown of the same.
Phase 2 (NEJM, 2023). In adults with obesity, average weight loss was calculated at 24 weeks and 48 weeks on several doses. The results were as follows:

TRIUMPH-1 (phase 3, May 2026). Among adults, average weight loss at 80 weeks was as follows:
A subgroup with a BMI (Body Mass Index) of 35 or above lost upto 30% at 104 weeks.
TRIUMPH-2 and TRIUMPH-3 (July 2026). In adults with type 2 diabetes, average weight loss was around 20.8% at 80 weeks on the highest dose.
In adults with cardiovascular disease, it was around 22.6% at the highest dose.
These are averages from controlled trials.
Participants had regular medical checks, correctly made medicine and supervised dose increases. None of that applies to a vial bought online.
Retatrutide is not a gentler weight loss option. At the highest dose in TRIUMPH-1:
The trials also reported dysaesthesia, an unusual skin sensation such as tingling, burning or sensitivity to touch.
In TRIUMPH-4, it affected 8.8% of participants on the 9mg dose and 20.9% of participants on the 12mg dose, compared with 0.7% of participants on placebo.
It was usually mild.
Longer-term safety, including heart safety, is still being studied. That is exactly what regulators will review before deciding whether to license it.

In July 2026, Eli Lilly said it plans to apply for US approval in early 2027.
A UK decision would need a separate application to the MHRA. No UK timeline has been announced.
Until the MHRA grants a licence, no GMC-registered prescriber can prescribe it as a licensed medicine and no GPhC-registered pharmacy can sell it. If and when it's licensed, we'll update this guide.
In August 2026, the MHRA issued a public warning about retatrutide being sold on social media and websites.
The risks are specific.
You don't know what's in the vial. Eli Lilly does not supply retatrutide to anyone outside its trials. So anything sold as retatrutide was made by someone else, with no quality control. It could contain the wrong amount, a different substance, or harmful contaminants.
It's often made in unlicensed facilities. In May 2026, the MHRA carried out its largest-ever seizure of unlicensed weight loss medicines at a site near Northampton. Around 12,000 doses were seized, including retatrutide and tirzepatide products, alongside packaging materials. Two people were arrested.
"Research use only" labels don't make it safe. Many sellers label retatrutide as a research peptide or "not for human consumption". That wording is there to protect the seller, not you. It means nobody has checked that it is fit to inject.
You mix and dose it yourself. Online retatrutide usually arrives as a powder that you mix with water and draw into a syringe. Getting the dose wrong is easy. A dose that's too high makes severe vomiting and dehydration more likely.
Nobody checks whether it's safe for you. A proper consultation screens for conditions and medicines that make weight-loss injections unsafe, such as a history of pancreatitis, certain thyroid cancers, pregnancy or planned pregnancy. An online seller asks none of those questions.
Storage and delivery aren't controlled. Peptide medicines can break down if they get too warm. Unregulated sellers have no cold-chain standards.
There's no one to turn to if it goes wrong. A regulated pharmacy has a pharmacist, a complaints process, and accountability to the GPhC. An Instagram seller has none of these.
